New Patient Registration
To register with the Practice, you must produce a Medical Card or complete a form known as a HS22X (you can obtain one at the reception desk) along with photographic identification and proof of residency, which are sent to HSC Business Services Organisation (Government Agency in Northern Ireland) who decide on your eligibility to register with a GP in Northern Ireland.
New patients will be invited to attend the Healthcare Assistant for their first appointment. She will take your Medical History and check your Blood pressure, weight and height. Inform the Healthcare Assistant of any medical concerns which you might have or any current medication which you must take (that is medication prescribed by your former GP) for an existing medical condition. New patients who do not attend their new patient appointment will not be registered or accepted anytime in the future by the practice.
You can also enquire about lifestyle and health promotion e.g. stop smoking plans, advice on reducing your alcohol intake and weight management.
The Reception staff will inform you of the registration procedure and answer any questions you might have.
Staff should also provide you with a copy of our Complaints Policy.
If you fail to keep your “new patient” registration appointment without explanation you will not be offered another appointment.
